Travel is occasional. Must possess a valid driver's license and must maintain required car insurance.
Preferred Minimum Qualifications
- A master’s degree and three years of professional experience in social work, child support, adult protective services, child welfare, early childhood development and education, guidance and counseling, nursing, intellectual disability, probation and parole, public administration, community-based prevention and diversionary youth services programs, juvenile justice, rehabilitation counseling, vocational evaluation or vocational rehabilitation;
- OR an equivalent combination and experience, substituting 1 additional year of qualifying experience for each year of the required education.
*Professional experience in the fields listed must have been the primary job responsibility to be considered as qualifying. Incidental performance of professional work in any area(s) shall not be considered.
Job Duties
The Programs Field Representative (PFR) will coordinate placement for criminal cases, working with the office of Public Guardian (PG) to coordinate placement for these individuals approved for Developmental Disabilities Services (DDS).
The PFR will serve as a liaison with the Public Guardian.
When new referrals are received the PFR will complete an assessment to ensure individuals are placed in settings that meet their needs and protect the public.
This position includes direct contact with individuals as they are evaluated for DDS services, assisting the case manager with the transition meeting and ensuring all needed services are in place before the move date. Will follow the case up to 90 days by attending the 30-, 60-, and 90-day meetings if applicable.
The PFR will coordinate placement searches for individuals meeting criteria to discharge from Acute Care facilities that are approved for residential services.
The PFR will document all efforts and report to the Program Manager.
The PFR will lead the team in all transition processes once a placement is found.
The PFR will coordinate placement searches for individuals coming through our intake unit. Assisting the intake case manager with locating placement options, scheduling meetings with providers and individuals. Then assist the community case manager with the transition meeting and ensure all the services needed are in place before the move date. Will follow the case up to 90 days by attending the 30-, 60-, and 90-day meetings if applicable.
The PFR oversees transitions, serves as liaison with state and area staff in the development and completion of transitions in the area, resolves transition issues as they occur, trains staff on the transition process, OKSA, and Community Transition Funds, while assisting to improve the Transition process.
The PFR will assist Case Managers in resolving complex consumer service issues.
The PFR is responsible for training the transition process in the Case Manager Academy.
The Program Field Representative will provide examples and instructions on the transition process. Provide tools and contact information for the Transition Coordinators.
